ARE YOU A NEW PATIENT?

We are always welcoming new patients here at Ambulatory Foot Care Center. We ask that you complete a few forms, including the medical history and patient registration forms. 

Also, please make sure you have your insurance card(s) on your first visit to our podiatrist office. It is also the patient's responsibility to bring their referral, if required, with them to our office.

Please arrive a minimum of 15 minutes prior to your appointment so you have time to complete the registration process. If you are a patient of ours already and have already completed the registration forms (by accessing them through our website or by picking them up at our office), then you only need to arrive 10 minutes before your scheduled appointment time.
